The Physician
Four decades at the bedside.
Charan Shikh, MD, is a physician with more than forty years of clinical practice in Internal Medicine, with a focus on longevity, preventive, and aesthetic medicine.
Most of that career has been spent in the ordinary work of internal medicine — blood pressure, blood sugar, cholesterol, thyroids, sleep, weight, the slow accumulation of small problems into large ones. It is unglamorous work, and it is where the real leverage over how a life ages turns out to live.
Over those years a pattern became impossible to ignore. Two patients of the same age, with similar labs, could be living entirely different lives — one still traveling, lifting, working, thinking clearly; the other narrowing year by year. The difference was rarely a single dramatic intervention. It was the accumulation of decisions made across decades, most of them modifiable, most of them made long before symptoms appeared.
That observation became a practice, and eventually a book.

Three convictions shape the work.
Function over numbers
Lab values matter because of what they predict about strength, clarity, and independence. The goal is never a prettier panel. It is a life that still works at eighty.
Evidence, stated plainly
Longevity attracts more enthusiasm than proof. Where the evidence is strong, it should be acted on. Where it is thin, that should be said out loud rather than sold.
Decades, not appointments
Aging is a forty-year process. It responds to what you do consistently across it, which means the plan has to be something a real person can actually sustain.
